Flying internationally with a prosthesis: what changes outside the US checkpoint

TSA procedures and the Air Carrier Access Act stop at the US border. A practical guide to what international security screening looks like, how battery rules work on international flights, and what to sort out before you board.

The TSA is a US government agency. Its jurisdiction ends at the US border. The Air Carrier Access Act is a US federal law. It applies to flights operated by US carriers, and to foreign carriers operating at US airports. Once you step off onto an international connection or board a non-US carrier at a non-US airport, you are working with a different set of rules, enforced by different agencies, with different consequences when something goes sideways.

Most of this does not matter most of the time. The x-ray machine looks the same. The screening officer asks similar questions. But some of it matters enough to be worth knowing in advance rather than discovering at a security lane in an airport where you do not speak the language.

This guide covers what actually changes, what stays the same, and what to sort out before you leave.


Security screening outside the US

Virtually every international airport uses the same basic security model: x-ray conveyors for bags, metal detection or millimeter-wave scanners for people, and secondary screening when something triggers the system. A prosthetic limb will trigger the system. That part is universal.

What varies is how secondary screening gets handled and what rights you have during it.

At most international airports, you can expect additional screening that involves either a manual pat-down of the prosthetic device and surrounding area, a swab of the device for trace detection, or both. You can ask for screening to be conducted by an officer of your gender. The option may not always be immediately available, but it is a reasonable ask.

Disability assistance services exist at most major airports globally, operated by the airport authority rather than a government security agency. These services can help you navigate security lanes, arrange for a private screening area, and coordinate with airline staff for boarding assistance. The level of service varies substantially. At Heathrow, Changi, or Schiphol, the service is well-organized and pre-registerable. At smaller regional airports, you may be explaining the situation from scratch.

If you want this assistance, contact the airport (not just the airline) directly before travel. Most airport websites have an accessibility or special services page with a contact form or phone number. Requesting it in advance is not mandatory, but it is how you get more than the baseline.

Carry documentation. A letter from your prosthetist on clinic letterhead describing the device, its components, and that it is a medically prescribed device is worth having in your carry-on. Not all airports will ask for it. At the ones that do, or in situations where language is a barrier, having the letter in hand accelerates the conversation. If your device includes a microprocessor, a lithium battery, or a powered component, the letter should describe those specifically.


Lithium batteries on international flights

Powered prosthetic knees, powered ankles, and myoelectric arm systems run on lithium-ion batteries. The rules that govern these batteries on aircraft are set by IATA, the International Air Transport Association, whose Dangerous Goods Regulations are followed by nearly every commercial airline globally.

The practical upshot of IATA’s battery rules for most powered prosthetics:

Batteries under 100 Wh can be carried in your carry-on without specific airline approval. Most powered prosthetic components fall into this range. A typical microprocessor knee battery runs in the 20-40 Wh range, well under the threshold.

Batteries between 100 and 160 Wh can usually be carried in your carry-on, but may require explicit airline approval in advance. This is an unusual range for prosthetic batteries specifically, but worth checking if your device uses a larger external power system.

Lithium batteries may not go in checked baggage unless they are installed in the device. A spare battery in your checked suitcase is generally prohibited under IATA rules. Spare batteries need to be in your carry-on, with terminals protected from short circuit.

How to calculate Wh if your battery is rated in mAh: multiply the milliamp-hours by the voltage and divide by 1,000. A 5,000 mAh battery at 7.4V is 37 Wh, well within the carry-on threshold.

Your device’s documentation or your prosthetist can confirm the battery rating. If you are unsure, ask before you fly. Airlines can and occasionally do turn back passengers with batteries they cannot document at check-in.

The FAA publishes traveler-facing battery guidance (linked above) for flights to, from, and within the US. IATA-based rules apply on international segments. In practice, they are very similar, but checking your specific airline’s lithium battery policy directly before departure is the right move, particularly for longer trips with multiple carriers.


Gel liners and the liquid rules at international security

Gel prosthetic liners can create a genuine problem at international security checkpoints that follow the 100-milliliter liquid restriction common at airports globally. A standard gel liner holds more than 100 ml of gel material. Some officers will flag it; some will not.

Under US TSA rules, medically necessary liquids, gels, and aerosols are exempt from the 3-1-1 rule, and prosthetic liners are recognized as medical items. The exemption is explicit in TSA policy.

International equivalents are less uniform. The EU’s harmonized security rules do include an exemption for medically necessary liquid items, but the procedures for invoking it vary by airport and country. In practice, a letter from your prosthetist describing the liner as a medically required prosthetic accessory is the most portable documentation you can carry.

When in doubt at any checkpoint, state clearly that the item is a medical prosthetic liner before it goes through screening. Do not wait for it to be flagged and then explain. Proactively describing medical items at the point of screening is how you get the conversation started on the right terms.

If you are using a roll-on or spray suspension aid that is liquid-based, pack it in a clear bag and have your medical documentation ready. Keep an extra liner in your carry-on in case one is held at a checkpoint. Losing a liner to a security dispute mid-trip and having no replacement is a preventable problem.


In-flight accommodations: what replaces the ACAA internationally

The Air Carrier Access Act requires US carriers and foreign carriers at US airports to provide certain accommodations: boarding assistance, accessible lavatories on aircraft with such facilities, stowage for assistive devices, and protection against discrimination. It does not travel with you onto non-US carriers operating outside the US.

EU Regulation 1107/2006 covers passengers with disabilities on flights departing from or arriving at EU airports, and on EU carriers for flights into the EU. It requires airports and airlines to provide assistance with boarding, deplaning, and connection, as well as stowage of mobility aids at no charge. If you are flying in or through Europe, this regulation applies, and you have a legal basis for requesting assistance and for filing a complaint with national enforcement authorities if it is refused.

Outside the US and EU, the framework is patchwork. Canada has a similar regime through the Canadian Transportation Agency. Australia has regulations through the Civil Aviation Safety Authority and the Australian Human Rights Commission. Many other countries have no enforceable equivalent.

In practice, most major international airlines will provide reasonable assistance if you ask in advance. Contact the airline’s accessibility services team when you book, not at check-in. Request the specific accommodations you need: a bulkhead seat with extra room, early boarding, device stowage near your seat. Put the request in writing and get confirmation in writing. At a gate in a country where your legal standing is unclear, what you have in writing from the airline is your practical negotiating tool.

On the flight itself, the most relevant considerations:

  • Battery-powered devices must generally be turned off or switched to “travel mode” if the manufacturer offers one during takeoff and landing. Confirm with your prosthetist whether your specific device has a travel mode and how to activate it.
  • If you need to use or adjust your prosthesis during a flight, doing so at your seat or in the lavatory is typical. Asking a flight attendant for a moment of assistance or privacy is reasonable.

Customs and medical device documentation

Carrying a prosthetic device internationally is carrying medical equipment across borders. Most countries do not require a permit or formal import documentation for a personal-use prosthetic device, but customs officers sometimes ask questions.

The most useful document is the same prosthetist’s letter described above, plus any device serial numbers or documentation from the manufacturer. For expensive powered prosthetics or devices with embedded electronics, a receipt or valuation document from your clinic may be helpful at customs if a device is new or clearly high-value.

For trips longer than 90 days or with frequent cross-border travel, it may be worth checking whether any specific entry documentation is required in your destination country. Most countries follow general medical device conventions, but a few (particularly those with restricted import categories for electronic components) have more detailed requirements. The US State Department’s country information pages and the destination country’s customs authority website are the right places to check.


If something breaks abroad

Prosthetic component failure during international travel is genuinely disruptive. The options depend on how far you are from a functioning O&P system.

Before you leave:

  • Get your device’s manufacturer name, model, and component serial numbers in writing and keep them accessible offline (a photo on your phone, a note in a travel document).
  • Ask your prosthetist which components are most likely to need replacement and whether those components are available globally or only through specific distributors.
  • Identify the manufacturer’s international support line or distributor network. Major manufacturers (Ottobock, Ossur, Fillauer, Proteor) maintain international presences and can sometimes facilitate emergency component access in major cities.
  • For powered devices, bring a spare charger and consider whether a spare battery (within IATA carry-on limits) makes sense for a long trip.

If something breaks during travel:

  • Contact the manufacturer’s international line first. They may be able to identify an authorized repair center in your region.
  • The International Society of Prosthetics and Orthotics (ISPO) publishes country-based member directories that can help identify credentialed local practitioners.
  • Major hospitals in large cities often have or can refer to orthotics and prosthetics departments.

Travel insurance is worth reviewing specifically for prosthetic device coverage before an international trip. Standard travel medical insurance typically does not cover elective prosthetic repair or replacement; it may cover emergency medical care related to a device failure that causes injury. Read the policy definitions carefully. Some travel insurers offer riders that expand medical device coverage; this is worth asking about specifically before purchasing.


The version of this that is actually a list

Before an international flight with a powered prosthetic:

  • Confirm your device’s battery Wh rating and check it against the airline’s carry-on battery policy.
  • Pack spare batteries and spare liners in your carry-on, not checked bags.
  • Get a prosthetist letter describing the device, battery, and any gel liners as medical equipment.
  • Contact the airline’s accessibility team and the destination airport’s accessibility services before travel.
  • If traveling through the EU, know that EU Regulation 1107/2006 covers you.
  • Have the manufacturer’s international support contact saved offline.
  • Check whether your travel insurance covers emergency prosthetic-related expenses, and what it excludes.
